If your lower back aches by mid-afternoon, or rolling over in bed has turned into a project, you're far from alone. It's one of the things expectant moms tell us most often here at Innate Chiropractic in Albany.

I'm Dr. Jenn Tran, DC, CACCP, our resident pregnancy and pediatrics specialist, certified through the International Chiropractic Pediatric Association (ICPA), and much of my week is spent helping East Bay moms move more comfortably through pregnancy.

In this post, I'll explain what's behind pregnancy back pain, how prenatal chiropractic can help, what care at our office looks like, and what kind of progress to expect along the way.

What Is Pregnancy Backpain?

Pregnancy back pain is aching, stiffness or sharp pain in the lower back, the back of the pelvis, or both, that starts or gets worse while you're expecting. Many moms notice it most when standing for a long time, climbing stairs, getting in and out of the car, or turning over in bed. Some feel a deep ache low in the back. Others feel it at the front of the pelvis, down into the buttock, or as sciatica running down one leg. These complaints tend to peak in the second and third trimesters.

It's very common. A large review in the European Spine Journal estimated that about 45% of pregnant women experience low back or pelvic pain.

Why does it happen? Your body is changing quickly. Pregnancy hormones such as relaxin loosen the ligaments around your pelvis to prepare for birth. Your center of gravity shifts forward as baby grows, and your posture adjusts to carry the extra weight. Moms who had back pain before pregnancy, or in an earlier pregnancy, are more likely to feel it again.

Why Prenatal Chiropractic Is One of the Best Treatments for Pregnancy Back Pain

Prenatal chiropractic works directly on the joints and muscles that are under new strain during pregnancy, and it does so without adding medication. For the many moms who want to limit medications while expecting, a gentle, hands-on option matters.

The key is that care has to be tailored. A pregnant pelvis behaves differently from a non-pregnant one, and it keeps changing from trimester to trimester. So I adjust my approach as you progress: lighter, lower-force adjustments, comfortable pregnancy-friendly positioning, and close attention to the sacrum and pelvis, where so much of the load lands.

Research on chiropractic for pregnancy-related back and pelvic pain is still growing. A 2020 systematic review in the Journal of Manipulative and Physiological Therapeutics found the evidence favorable but not yet conclusive. That's why every plan starts with an individual exam. What I see in practice is moms moving and sleeping more comfortably when care fits their body and their stage of pregnancy.

What Does Prenatal Chiropractic Do for Pregnancy Back Pain?

Here's what moms most often notice with prenatal chiropractic care at our office:

  • Less aching in the low back and the back of the pelvis
  • Easier everyday movement, like walking, stairs and getting out of the car
  • More comfortable sleep and easier turning in bed
  • Better posture as your body adapts to a growing baby
  • A clear plan for staying active through each trimester

Many moms also tell me they simply feel more at home in their bodies from week to week. Every plan is built around your exam and how far along you are. Learn more about our pregnancy care program at Innate Chiropractic.

What a Prenatal Chiropractic Program Might Include

No two pregnancies are the same, so no two plans are either.

Most prenatal programs at Innate Chiropractic are built from three core pieces.

What a Prenatal Chiropractic Program Might Include:

1. A thorough Patient Exam

2. Gentle Adjustments and the Webster Technique

3. Whole-Pattern Care and Support at Home

A Thorough Prenatal Exam

Your first visit starts with a pregnancy-specific history and postural evaluation. I'll ask how far along you are, where you feel pain, and what makes it better or worse. Then I gently assess your low back, pelvis, hips and sacrum to see where the strain is coming from. My care works alongside your OB or midwife, not in place of them, and I'm happy to coordinate with your birth team.

Gentle Adjustments and the Webster Technique

Adjustments are low-force and modified for each trimester. As your belly grows, I use side-lying positioning so there's never any pressure on your abdomen. I'm certified in the Webster Technique, a specific protocol for balancing the sacrum and sacroiliac (SI) joints. That balance matters, because pregnancy back pain usually starts with the pelvis and sacrum shifting as your ligaments loosen.

Whole-Pattern Care and Support at Home

Pain rarely stays in one spot, so I treat the whole pattern: pelvis, sacrum, low back and round ligament tension, not just the area that hurts. I'm also certified in Dynamic Body Balancing, a gentle approach I use alongside adjustments. Between visits, I'll share simple sleep-position tips and everyday posture habits. If you'd like, care continues through delivery and postpartum with the same provider who knows your history. You can read more about how we approach low back pain at every stage of life.

The goal is a plan that feels manageable and actually fits your week, so you can keep up with work, family and getting ready for baby.

Managing Expectations: Progress Takes Time

Pregnancy back pain usually doesn't disappear after one visit, and that's normal. Your body is changing every week, so care is less about a one-time fix and more about keeping you comfortable as those changes happen.

In the first few visits, many moms notice short stretches of relief: an easier morning, a better night's sleep, or less stiffness getting out of the car. Over the following weeks, those good stretches tend to last longer as your pelvis and low back adapt. Some weeks will feel better than others, especially as baby grows or shifts position.

That's why I'll check in with you at each visit and adjust your plan as you move through each trimester. Staying consistent, keeping up with your home stretches and telling me what's changing are the things that help most.
